The Parent Exclusion Form excludes students from taking the state required tests. These tests have student data that contributes to school accountability. Since these tests are also used at the school level to inform individualized instruction, we do require that students who don't take the state required test to take a school level test (NWEA) that will give the instructional data to the school that we need for each student but is not shared with the state. The NWEA tests the same subjects in an online format that is similar in time and scope as the state required tests, so we can get as close as possible the same level of data to know the instructional needs of all students at Career Path High.
Students who are not participating in state testing will need to attend NWEA testing on May 10th and May 24th at 9 AM.
Please use the button below to access the exclusion form from the state website. All forms are due prior to the beginning of testing.
