This is the first step in receiving assistance for inquiries related to counseling or support services

As a blended learning model of education we are able to provide services to our students from any location. In the same manner that instruction is delivered to our students, you and your student are able to receive assistance with your inquiry requests without coming to our physical location.

We want to help you and meet your needs. In order to do so in an efficient and timely manner we utilize the following process. Please go HERE to complete an Inquiry Request Form, enter the appropriate information in the fields, and click submit. It’s that easy. Assistance with scheduling, credits, DATC transmittal forms, computers or any inquiry related to counseling or support services are initially addressed through this process. Your request is then sent to the CPH team member who can best address your inquiry. If your request requires a face-to-face meeting, you will be contacted to schedule an appointment.

Please note: If you drop by the counselor or main office you will still be required to follow the process for the Inquiry Request Form.

If you have an inquiry specifically for an Instructor or Success Coach, do not fill out the Inquiry Request Form. Instead, please contact them directly using the contact information found on our website under People/Faculty Contact Information.
